Robert is a 62 year old retired police detective. Around 50, he started noticing some symptoms that impaired his ability to fulfill his work duties and went on work leave sighting PTSD. As the symptoms developed rapidly, he was diagnosed with early on set dementia. His family estranged themselves from him and his mum and dad, not having any solid understanding of supports available, placed him in an aged care facility, dementia unit.

This was a shock to Robert and he declined with the new behaviors emerging from poorly matched care. His mum and dad who were in their 80s had to drive over an hour each way to visit.
Roberts father passed away and his mum was eager to have her son closer.
In consultation with Robert’s aged care advisory team NDIS Planner, we mapped out a plan to get Robert in a better situation for him and his mum. The process started with a change of situation sighting the desire to relocated to SIL accommodation supported with support need evidence from a comprehensive FCA and SIL assessment completed by his known OT and a home and living application.
Following a review meeting, a new plan was provided allowing cross over funding to the aged care facility and to any new potential SIL providers, for trial or to temporarily cover a move in. We were able to receive support from an NDIS representative from the home and living branch with suggestions about potential providers. We also had prior approval from the home and living branch for 1:3 ration SIL funding. We found a registered organization called LINK2U who were prepared to work with Rob’s mum to secure a rental property 3 streets away from her own home. Robert was the first person to move in and the transition went brilliantly. He has now linked with local OT and BSP services to assist him in his new environment. He has also been able to maintain his weekend NDIS Community Participation worker that supported him in the aged care facility. His mum walks up to see him every day 😊.
