Getting to know Dan

Meet Dan

Dan is a 16 year old twin who lives with his brother, mum and nan. He goes to a local support school and as he went through puberty and grew in size, his challenging behavior became unmanageable with several episodes of police involvement and hospitalisation.

The Background

Dan had a small NDIS plan with some core funding and therapy. Mum had not been able to link to him supports due to the severity of the day to day management of the behaviors and no support coordination.

Our Solution

We first became aware of Dan when he was in hospital, in the acute mental health unit where he underwent a comprehensive medical and psychological review. We supported his family to engage with an OT for an emergency OT assessment and a psychologist to report on the need for BSP using the funds that were available at the time. We supported the family to link the health and NDIS systems to get Dan a quick review and excellent plan.

Outcome

The new plan included level 2 and level 3 support coordination and Dan’s mum chose Auscare for Support Coordination and Plan Management.

Support coordination conducted an implementation meeting with Dan’s mum to map out the best way to ensure choice and control with creating a better happier life for Dan and his family.

Dan has matched well with support workers and clinicians, who are assisting him to explore his very clever creative musical talents so he can be the best version of himself. He also has regular respite to ensure family preservation.
