What is Support Coordination?

From short-term help identifying and managing supports, to ongoing assistance coordinating providers and building independence, your Support Coordinator works with you to find the right providers and boost your skills over time. How to get started …
1. Request it in your planning meeting
Support Coordination isn’t automatically included in your NDIS plan, you need to ask for it at your planning or review meeting.
2. NDIA assesses if it’s right for you
They will consider how new you are to NDIS, your current supports, any major life changes, and if coordination will help you build skills and confidence.
3. Get matched with your coordinator
Once approved, we will connect you with a coordinator who understands your situation and goals.
4. Start building your supports
Your coordinator helps you connect with providers, set up agreements, and navigate the system with confidence.
Typically funded at 1-9 hours per month depending on your needs, reviewed plan by plan.

Can I get funding for Support Coordination?
Support Coordination is not automatically included in your NDIS budget, so you will need to request it at your planning meeting. The NDIA determines if it is ‘reasonable and necessary’ based on your situation, including how new you are to the NDIS, your informal supports, and any major life changes.
Where are your Support Coordinators located?
We have Support Coordinators located right across Australia that can provide face to face or remote support. What ever you would prefer.
How do I prepare for my planning meeting?
Be ready to detail how Support Coordination will help you build capacity and achieve your goals. Your coordinator can help you prepare the documentation you need.
What if my needs change?
Life happens. Changes like starting employment, moving out of home, or shifts in your support needs may mean your plan needs reassessment. Your Support Coordinator can help you lodge a change of situation form.
